How to fax letter of recommendation to Columbia College?
I’m just curious- anyone who has recently applied to Columbia College in Chicago- how do you go about faxing a letter of recommendation? There is a form that needs to be filled out with your info in addition to the letter itself, which needs to have your name and social security number on it. Do I just fax the both of these to the number it specifies and I’m done?

Well you have a choice. You can just have your recommender fill out the provided form and fax that in. Or they can write up their own letter and in that case, you need to fill out your information at the top of the form and the recommender fills out section one (which is where they place all of their information i.e., name, title, etc…) -you then fax both the form and their written letter to the admission office.
After you fax it, call the office to verify they received it..just to be sure!
Duh
